2 min success story: What is World class care!! - Massive Anterolateral MI (heart attack) - Complicated by Ventricular fibrillation on way - EMS begin CPR within seconds, transports clinic within minutes - Over 90 Minutes cardiopulmonary resuscitation (of note: With every minute delay, Chances of recovery are reduced by 7-10%... Within 4-6 minute Brain damage and permanent death start to occur... After 10 minute Few attempts at resuscitation succeed!!!! .... Basically a 10-15 min delay in beginning CPR will lead to Death) - CPR continues BEFORE, DURING & AFTER the catheterization! - Meanwhile heart supported on :- IABP (The Intra-aortic balloon pump (IABP) is a mechanical device that increases myocardial oxygen perfusion while at the same time increasing cardiac output. Increasing cardiac output increases coronary blood flow and therefore myocardial oxygen delivery ECMO (extracorporeal membrane oxygenation (ECMO) is an extracorporeal technique of providing both cardiac and respiratory support oxygen to patients whose heart and lungs are so severely diseased or damaged that they can no longer serve their function) IMPELLA (Impella® is a temporary device used to aid the heart in pumping blood when a patient’s heart is determined too weak. Impella® is the world’s smallest and only minimally invasive, mechanical cardiovascular support system. The Impella® is a tiny pump inside a catheter, only 6.4 mm in diameter. It has an electric motor that can pump approximately 2.5 liters of blood per minute sustaining the heart for up to 7 days) - sustained SEVERE ANOXIC BRAIN INJURY - kept in CICU for 2 days, like paralyzed, no one thought his brain could recover!!, HE KEPT BREATHING, TOUGH GUY!! - transferred to regular floor (where i saw him, thinking he might need palliative care and hospice ie end of life care) - MIRACLE DO HAPPEN, great recovery, day 3 on floor, pt Alert oriented to time place person, Glasgow coma scale 15/15, taking normal diet, walking with assist, getting physical therapy and awaiting rehabilitation placement, in a week will be at home. Following the ACLS protocols and evidence based medicine, understanding the importance of every second of critical life, from paramedical staff on EMS team, to ED physician, staff, and nurses, to Cath lab team, CICU, then floor.... Every one was incredibly outstanding, providing world class care .. And this makes CLEVELAND CLINIC .... WORLD #1 hospital for Cardiovascular medicine!!!! I wish everyone in this world can get the same care, as EVERY LIFE DESERVES WORLD CLASS CARE!
