A Minority of doctors cause most of the harm, and of those who cause most of the harm there is little effort to do anything meaningful to stop them. In a perfect world it would be nice to believe that hospitals want to do the right thing. This is a slippery slope when we already see how dishonest hospitals have behaved in situations where a doctor has been known to cause harm. If disclosure helps the good doctors speak up and get rid of the incompetent liars, this might help. Otherwise, I am afraid risk managers will use a program to keep more secrets from the public and the courtroom. There is a way... Remove Risk Managers from the Equation.
