Position Care Navigator POSITION SUMMARY: Enroll new consumers into the Health Care Home Program and administer the baseline assessment, reassessment and discharges. Work with both the RN and primary care doctor to follow up on consumer referrals to specialist, and assist the consumer with scheduling the appointment. Assist with the outreach to recruit new consumers. Complete data tracking in both TRAC system and internal system for quarterly reports. Assist consumer and entire team with managing integrated treatment for improved health outcomes. Act as the liaison between consumer, providers and internal staff. Co-facilitate integrated group. Track and document all necessary data as instructed by the Director. ESSENTIAL FUNCTIONS: • Demonstrates and reflects a commitment to the mission and values Milestone by serving others with respect, dignity and compassion. • Assist consumers in Healthcare Home Project to navigate the healthcare delivery system. • Enroll consumers into the Healthcare Home Project. • Contacting insurance company to transfer PCP to SHHC. • Follow up on referrals made to specialist. • Establishes effective relationships with specialists, health plans, healthcare facilities. • Assist with data collection, tracking and enter data into clinical registry tracking form. • Participate in integrated treatment team meetings. • Participate in program evaluation. • Develop effective relationship building with other department teams. • Develop resources and effective relationships with consumers. • Provide coaching and support. • Familiarity with local community resources and provide those to consumers. • Facilitate groups and back up for wellness groups. • Coordinate treatment between mental and physical health. • Administer health indicator screenings and schedule quarterly screenings. • Other duties as assigned. MINIMUM QUALIFICATIONS: Master’s degree in Social Work or related human service field. LSW preferred. Experience working in both physical and behavioral health settings. Must have valid PA license.
