The humeral shaft has anteromedial, anterolateral, and posterior surfaces. Proximal and midshaft fractures are more amenable to plating on the anterolateral surface:, whereas distal fractures often require posterior plate fixation So .... Proximal and middle-third humeral shaft fractures are addressed using an anterolateral approach. Distal-third humeral shaft fractures often are treated via a posterior approach, because the distal humeral shaft is flat posteriorly, making it an ideal location for plate placement.
