Workplace Health and Safety Queensland is investigating a fatal incident that occurred on Thursday 22 August 2013 at Mt Perry in Central Queensland. A farmer died when the end loader he was driving failed to negotiate a bend on the road, slid down an embankment, then rolled over and crushed the operator. Could this happen in your workplace?
