Soufriere carnival pageant 2014 REQUIRMENTS • Be a citizen of Saint Lucia • Be between 18 to 25 years • Not be married and have never been married • Not be pregnant and have never borne a child • Possess a minimum of 5 CXC passes at grades 1, 2 and/or 3 (Including English & Mathematics) • Be able to perform a talent of high standard (e.g. Dance, Drama, Music etc) SUPPORTING DOCUMENTS REQUIRED: 1. Completed form (page 1) 2. Two photographs - one (1) 8 x 10 colour photograph (Portrait) and one (1) colour photograph in swimwear (Full body) 3. A police record (this should be dated at least one month prior to the application date) 4. Valid picture ID 6. A brief description of your reason for applying to be a contestant in the Soufriere Carnival Queen Pageant( Page 1) 7. A short biography (100-150 words) (page 2)
