1 UNIVERSITY OF DAR ES S A LAAM OFFICE OF THE DEPUTY VICE CHANCELLOR ACADEMIC DIRECTORATE OF UNDERGRADUATE STUDIES INSTRUCTIONS TO REGISTRATION OFFICERS 20 1 3 / 201 4 ACADEMIC YEAR 1. WHO SHOULD REGISTER All f irst y ear s tudents admitted into the Universi ty for the a cademic y ear 20 1 3 /20 1 4 should register at the ir respective College/ School /Institute on the dates indicated on the Orientation Programme. 2. WHERE SHOULD THEY REGISTER Students from the Mwalimu J.K. Nyerere Mlimani Campus should register at the Mwali mu J.K. Nyerere Mlimani Campus ; Students fro m D ar es Salaam U niversity C ollege of E ducation (DUCE) should register at DUCE; Students from M kwawa U niversity C ollege of E ducation (MUCE) should register at MUCE ; and Students from S chool of J ournalism and M ass C ommunication (SJMC) should register at S JMC . 3. ITEMS TO BE CHECKED BY THE REGISTRATION OFFICER All s tudents should submit to the Registration Officer t wo sets of Registration Forms. Registration Officers should not accept incomplete registration forms . They must check each and every item on the Registration Forms very carefully and ensure that they are in order and correct . Special attention should be given to the following items: (i) The Photograph that appears on the Student’s Registration Forms should match the face of the person presenting the Registration Forms . (ii) The Registration Number indicated on the Registration Forms should be the same as that appearing on the Student’s A dmission L etter. (iii) The College/ School/ Institute/ Department and Degree Program me appearing on the Student’s Registration forms should be the same as that appearing on the Student’s Admission Letter. (iv) The Names appearing on the Registration Forms should match the ones ap pearing on the O - level and A - Level /Diploma Certificates as well as the Admission Letter . No additional names will be accepted . If there is a difference in names between the admission letter and A - level /Diploma certificate, the case should be referred to the Admissions Office. (v) The Certificates submitted for ver ificati on , i.e. birth, C.S.E.E. and A.C.S.E.E. should be originals as well as photocopies . Where ver possible , s tudents should submit Cert i ficates and not Results Slips . A - level Results Slips are only accepted if the Student has comp leted Form VI in the same year. Results Slips are not acceptable for O - level results . Ugandan students should bring certificates issued by the Uganda National 2 Examination Board (UNEB) and not testimonials from their Secondary School Headmasters/Mistresses. The Registration Officer shoul d certify the photocopies of the certificates and then attach them to the student’s registration forms. In case of any doubts, please refer the students to the Admissions Office. (vi) Form “ B ” should be submitted togeth er with the Registration Forms. Registra tion Officers should not accept Registration Forms without duly - filled Form “B” . (vii) Confirmation of Fee Payment – item no. 23 o n the Registration Form should be signed and stamped by the Bursar or College/School Accountant ( on his behalf) before the Registr ation Form can be accepted by the Registration Officer. (viii) Signature – The Regist ration Officer must write her/his full name and sign on behalf of the Director of Undergraduate Studies on page 4 of the Registration F orm. 4. STUDENT IDENTITY CARDS Only tho se students who have completed registration formalities will be issu ed with Student Identity Cards. 5 . REGISTRATION PROGRAMME Registration of Students admitted in to the various academic units will be conducted as per the Orientation Programme issued by th e office of the D irector of Undergraduate Studies. Note: Registration Officers should abide by the registration time - table by ensuring that they are present on the date, time and venue specified in order to avoid unnecessary complaints and long queues by the students.
