38 weeks, 2.6 kg, AGA male neonate referred on day 2 with respiratory distress since birth. Echo done by pediatric cardiologist showed PPHN. Baby maintained saturation on head box oxygen till day 3 when he was ventilated, sepsis screen negative at admission and on day 3. Baby was on full feeds by day 4. Initially baby was started on 22/5, FiO2- 0.8 and gradually weaned to FiO2 of 0.6 by 48 hours. Then baby started to have increasing ventilator requirements and CXR showed a whiteout. Baby was put on antibiotics on day 4 of ventilation with CRP of 40 mg/L which dropped to 15 mg/L over next three days. Xray continued to worsen but baby hemodynamically stable, on 150 ml/kg feeds. Whats going wrong
