A 38-year-old woman presents with recurrent chest infections. For some time she has noticed that her nails are yellow and misshapen and that she often has oedematous legs after standing all day in the shop where she works. Examination reveals evidence of lower lobe consolidation and possible pleural effusion. This is confirmed on chest Xray.
