A 42-year-old farmer is admitted with a severe cough, fevers and shortness of breath. You understand that he has been clearing out a shed over the past few days which were previously used for over-wintering cattle. On examination he has a temperature of 38.2C, his BP is 110/70 mmHg and his pulse is 95 and regular. There are widespread coarse crackles on auscultation. Investigations show: Haemoglobin 13.9 g/dl (13.5-17.7) White cell count 12.8 x 109/l (4-11) Platelets 222 x 109/l (150-400) Sodium 138 mmol/l (135-146) Potassium 4.1 mmol/l (3.5-5) Creatinine 110 micromol/l (79-118) CXR - diffuse consolidation.
