A 55-year-old diabetic man 8-days post liver transplantation has persistent hyperkalaemia requiring treatment. His medications include tacrolimus and insulin. His creatinine is normal, his pH 7.40, but his bicarbonate 12 and base deficit -10. Urine pH was 5 and serum lactate 0.5. Haptoglobins were normal. Direct Coombs test negative. Liver function tests: Bilirubin 28, ALP 350, ALT 58. The MOST likely cause of his hyperkalaemia is: A. Post transplant haemolysis B. Acute rejection C. Renal tubular acidosis D. Enoxaparin use E.
