A young man presents to the A&E department with an acutely swollen and painful right knee associated with red gritty eyes and dysuria. He has recently returned from Thailand where he had diarrhoea and vomiting for several days. Joint aspiration shows the presence of giant macrophages. No organisms are seen on Gram staining.
