ATHLETIC NEWS - FALL SPORTS: Sports Physicals MUST be completed and turned in no later than Wednesday, August 14 before your scheduled practice in order for your student athlete to participate in any practices; please note that this form requires a physical by a doctor (if you cannot get in to see your doctor by this date you can check with a med station). You can find the MHSAA Sports Physical form on our school website under the Athletic Tab/Forms. The other required athletic documentation can also be found under the Athletic Tab/Forms. These documents are: Athletic Participation Form, MHSAA Sports Physical Form, Athletic Code/Signature Form, Transportation Waiver, Concussion Awareness-Educational Material Acknowledgement Form, and the Athletic Fee. These can be brought to the Elementary Office, Monday – Thursday, 9am-2pm.
