Accident Assessment: A 52-year-old male diver was spearing lionfish for personal consumption. He wore a glove on his right hand with which he held the spear. After spearing a lionfish, he went to place the fish in a catch bag held in his ungloved left hand. He immediately felt intense pain as a pectoral spine punctured his left hand at the base of his thumb. The diver was able to maintain control, despite the pain and made a safe ascent to the surface. Once back onboard, several individuals offering suggestions for treatment. The first was applying an instant cold pack, the next was applying an instant hot pack and the last was using vinegar. Which treatment would be the most appropriate and why?
