Accident Assessment Answer: Skin abrasions in a marine or watery environment can sometimes have a torpid evolution. Regardless of how pristine water may look like, it is filled with microorganisms, and some of them can become opportunistic pathogens. Aquarium granuloma (fish tank granuloma or swimming pool granuloma) is a relatively common skin infection that affects individuals who work in watery environments. It is caused by an atypical bacteria (Mycobacterium marinum) that takes longer to develop lesions and is not affected by the type of antibiotics typically prescribed for ordinary skin infections. Furthermore, this bacteria will not show up on ordinary culture media, which is why the diagnosis is usually delayed. Divers should clean skin abrasions from any watery environment thoroughly and apply antiseptics generously before bandaging the wound. Always seek a professional medical evaluation afterwards, and if the injury still shows a torpid evolution after a week or two, consult your doctor and inquire about aquarium granuloma. If you or your doctor think DAN can be of any assistance, we are here to help.
