Step 2: Fill-up the Admission Form with the correct information. (Write legibly.) Step 3: Proceed to the Guidance Office with the duly filled-up Admission Form for some other pertinent personal data needed, then to the School Clinic for medical assessment. Step 4: Seek for the recommending approval of the Related Subjects Department Head, then the approval of the Principal. Step 5: Then, finally submit duly approved Admission Form to the Guidance Counselor.
