The City of Chicago is in the process of developing a new 2015-2019 Consolidated Plan for the use of Community Development Block Grant (CDBG), Emergency Solutions Grants program (ESG), HOME grant program, and Housing Opportunities for People with AIDS (HOPWA) funds. The City of Chicago is asking residents and service providers to provide their perception of critical needs in the community. Therefore, two online surveys have been created for residents and service providers to provide this feedback. The survey is open from July 16, 2014 until 8 p.m. on Monday, August 18, 2014.
