And they call it greedy doctors or criminally negligent medical error or other such fancy names----- Our job is not so simple guys. We walk on a very thin edge, with danger of falling over this way or the other, lurking at all times. Yes, greed and criminal element exists too, at times, I concede. Consider this example---- Chronic Renal Failure, Hypertensive, Post kidney transplant rejected patient, on maintenance Hemodialysis till Retransplant is feasible, Hepatitis B and C Positive and treated by Interferons and antivirals, operated 4 months back by me for disseminated intra-abdominal hydatid cysts (a foolish surgeon did a half hearted laparoscopic job on this dangerous pathology, while another foolish surgeon jabbed needles into the recurrent cyst, thus further disseminating it) landed up with lower GI bleeding. Generally we do Colonoscopy under sedation, but here, we had two hurdles----- sedation could not be even considered in view of the precarious medical condition, while there was severe cost constraint too, thereby a formal anaesthetist could not be paid for. Colonoscopy is a painful investigation and we always do it under conscious sedation. We started the procedure and as expected, the patient was in severe discomfort. Balls to the risk, we went ahead with our routine sedation protocol to alleviate his discomfort and not persist with continuing, as I see most colleagues doing (I have been a spectator to some of the most criminally painful resorts of interventionists upon patients. ANY INTERVENTIONIST WHO CAUSES PAIN, IS NOT WORTH HIS NAME----- you would never have seen me resorting to ALL CAPS as it is not the preferred mode of type/ written expression and is akin to visual shouting. I am shouting on this indeed, to drive my point home). Good intentions are rewarded generally by a good outcome. It was cakewalk and nothing happened. God is great!
