Date: Saturday October 18, 2014 Location: 502 Prospect Ch. Rd., Albemarle, NC 28001 in the upper parking lot Hours: 9:00AM-2:00PM You will experience the same quality, compassionate care, expertise and comfortable atmosphere, including digital mammography and female technologists certified in breast imaging. Billing Options EXAMS ARE NOT FREE. However, Charlotte Radiology is in network with most insurance plans, including Medicare and Medicaid, and will file insurance on your behalf. Please check with your carrier to determine coverage prior to your exam. If you are uninsured, a discounted self-pay fee of $150 is due at the time of your appointment. Or you may fill out the attached document in full and fax or e-mail as indicated on the form. Please have your physician’s name and insurance information available at the time of your call. Charlotte Radiology provides confidential results to all patients and their primary care physician(s) of choice. It is important to schedule your appointment before the mobile visit. A minimum of 20 women must be scheduled in order for the mobile to visit a location. Individuals will be given their choice of appointment time until the schedule is full. Walk-ins will be accepted on a very limited basis on the day of the event. Charlotte Radiology’s eligibility guidelines for screening mammography • Must be a woman 35 and older. Physician referral not required but must have a physician’s name for reporting purposes. • Women under 35 with a family history of breast cancer (mother or sister with breast cancer before age 50) are eligible. Physician referral is required. • Must be one year since previous mammogram. • Must not be pregnant or breast-feeding. • No new breast lumps/concerns. Patient should promptly contact their physician for evaluation if experiencing breast problems. How should I prepare for my exam? • Please bring your photo id and insurance card (if applicable) to your appointment. • If prior mammogram(s) were performed at a facility other than Charlotte Radiology, please bring those films to your appointment or the name and address of the facility where they were performed. • On the day of the examination, do not wear talcum powder, deodorant, lotion or perfume under your arms or on your breasts. These substances can cause artifacts on your mammogram making the images harder to interpret. Deodorant will be available to you after your exam. • Wear two-piece clothing so that you only have to remove your top and bra for the examination. • Please bring any pertinent history to your appointment: prior surgeries, hormone use, family or personal history of breast cancer. Charlotte Radiology and the American Cancer Society recommend annual mammography for women ages 40 and older. If insurance allows, a baseline mammogram is recommended for women between the ages of 35-39.
