Congenital lobar emphysema: 4 months (5kg) male infant with persistent respiratory distress since birth. X ray and Ct chest showed marked distension of the left upper lobe causing marked shift of the mediastinum to the right side and compression of the contralateral lung (almost herniating left lung behind the sternum to the opposite side). No history of foreign body inhalation ( however preoperative on table bronchoscope was done). Characteristically the affected lobe bulges out of the thoracotomy wound being markedly voluminous . Treatment in symptomatic infants is surgical by removal of the affected lobe (left upper lobectomy in this case)
