Counting down to ICD-10 259 days. That’s the time remaining until we start using ICD-10. How far along are you? You’ve completed your coding assessments, right? Hopefully, you’ve already started your coder training. At the very least, you should have brushed up on your anatomy and pathophysiology and started reviewing the ICD-10 guidelines. Do you know your top diagnoses and procedures? You can determine this by looking at the volume or by the reimbursement/costs. Look to see what percentage of your patient population is included in those top diagnoses and procedures. If your top 20 MS-DRGs only cover 30% of your patients, you need to expand your list. Have you audited your physician documentation? I know, you can’t wait to train your physicians. First you need to know what you need to train them on. If they’re already documenting laterality, don’t discuss it. Focus the training on their particular shortcomings. Show them their actual documentation and then show them the codes. Also, remind them that you didn’t invent ICD-10 to make their lives more difficult. ICD-10 should improve patient care by providing better information about the patient’s health. If you have physicians who say their patients are sicker than everyone else’s, therefore they have to provide more care, now they can prove it. Are your testing plans in place? If not, contact your vendors and find out if you still have time to test. Some vendors may have limited openings remaining because they’ve scheduled testing with all of their other partners. What are your plans for October 2, 2014? The transition doesn’t end October 1. You still need to audit your ICD-10 codes, identify any breakdowns in billing submission, track your reimbursement, you know, all of the things you do now. The transition to ICD-10 will not be seamless. Not all of your plans will work. Be ready to deal with the speed bumps. Ask for help. Talk to other facilities and practices. Network through state and national associations. Create your plans, then plan for contingencies.
