Birth Certificate with documentary stamps If birth certificate is not available due to loss or destruction of the Records of Birth of the Office of the Local Registrar, the following should be submitted. ____ a. Certificate of the Local Civil Registrar to the effect that petitioner’s certificate of birth is not available due to loss or destruction of the Records of Birth of the Municipality (with documentary stamps); and ____ b. Joint Affidavit of Birth executed by two (2) persons who have personal knowledge of petitioner’s date and place of birth, the name of her/his parents and their citizenship (with documentary stamps).
