Determining appropriate therapy to treat hyperglycemia in diabetic patients who have renal insufficiency (RI)/chronic kidney disease (CKD) is complicated by the fact that most available agents are cleared by the kidney and thus cannot be used in these patients or must be carefully monitored and frequently adjusted. Unfortunately, the few agents that are suitable for patients with RI/CKD—such as insulin and sulfonylurea—greatly increase the risk of hypoglycemia. Therefore, until recently, options for treating diabetes in patients with RI/CKD have been very limited.
