EMPLOYMENT OPPORTUNITY De dwa da dehs nye>s Aboriginal Health Centre Is currently Seeking: Aboriginal Mental Health Youth Patient Navigator (Full Time) - Hamilton & Brantford This Aboriginal Patient Navigator role is pivotal in the expanded care for child & Youth Mental Health Service. This role will assist the population identified to receive the organizational and community supports and Services in both Hamilton & Brantford. The target populations to be served are the aboriginal youth aged 17 to 24 years old. This navigation role will work mainstream and Aboriginal Service providers to help Aboriginal youth, Caregivers, and their families better navigate the Mental Health services to better improve patient outcomes. The Patient Navigator will be provide an early identification and intervention for concurrent disorders in youth and link the Aboriginal Youth with specialized services for Mental Health and Addictions where available and to explore linkages that provide similar services. As well, it will increase satisfaction between services and to help identify and reduce barriers to services and supports. SUMMARY OF RESPONSIBILITIES: The Aboriginal Patient Navigator will provide an increase ease of access to organizational and community supports and services including social determinants of health which have an impact on an individuals Mental Health, such as: • Identification (status, birth certificates, etc.) • Housing • Food security • Employment/Training • Cultural Resources • Life skills • Budgeting • Navigating the system (educate them to navigate the system) • Navigation of shelters • Access to Culturally Appropriate Services The Aboriginal Youth Patient Navigator will help Aboriginal youth reconnect with the community and Traditional Healing, especially those who are crown wards or who have been adopted providing a support system similar to the traditional family unit. The Aboriginal Patient Navigator will help alleviate the trust issues commonly found in the target population, as well as promote a holistic approach to providing specific services for youth. Day to day areas of responsibility include, but are not limited to Case Management and System Navigation • Support clients, caregivers and families to communicate with Traditional Healers, Elders, knowledge keepers, etc., including Physicians, Nurses and other members of the health care team. Education and Promotion • Assists with development of marketing and promotion strategies specific to the Aboriginal Mental Health Youth Patient Navigator role and services to our Aboriginal communities Operations • Ensure Information is gathered and data is collected daily QUALIFICATIONS AND REQUIRED COMPETENCIES: • Mental Health care professional in good standing with their provincial regulatory body, or lay Mental Health Liaison worker who is recognized by the Aboriginal Community; • Knowledge of case management/system navigation models and principles and minimum two years experience in an Aboriginal health and/or social service organization, hospital or community-based case management, Mental Health, Health Promotion, public health or social work is essential; • Knowledge of community resources, specifically Aboriginal programs and services in the HNHB LHIN catchment area; • Previous experience/interaction with the HNHB/Mental Health Services Providers and regional/tertiary health centres would be an asset. • Knowledge of tools, systems and databases used in client services delivery and case management. • Experience working with and extensive knowledge of Aboriginal Peoples language, culture, customs and traditions • Excellent written and oral communication skills required • Valid G drivers license and access to reliable transportation Closing date: Friday, January 9, 2015 at 4:00pm Please forward your application, references, and copies of required certifications to: ATTN: Tara Williams, Manager of Advocacy & Outreach Services De dwa da dehs nye>s Aboriginal Health Centre 678 Main Street East, Hamilton, Ontario L8M 1K2 Email: [email protected] Preference will be given to qualified applicants of Aboriginal descent.
