Events in Hudson County NJ come to mind. Also, Puerto Rico having to reissue all of its birth certificates due to massive forgery and fraud in that commonwealth. The birth certificates, both short-form and long-form alleged issued by the Dept of Health in Hawaii proffered by Barack Obama to the American electorate are both forgeries.
