FALL SOCCER REGISTRATION In Person Dates: Wednesday, July 16th (6pm - 8pm) Saturday, July 26th (10am – 2pm) Location: Brooklyn Community Center, 31 Tiffany Street, Brooklyn Mail-in registration: Mail-in registrations with payments included and post-marked on or before July 28th may be sent to: Brooklyn (CT) Youth Soccer, Inc., P.O. Box 435 Brooklyn, CT 06234 Ages: 4 (by October 31st) to 18 years old may register What You Need: 1. One player registration form for each child (Please make sure you are using the form with (Revised 6-26-2014) at the bottom); 2. Copy of child’s birth certificate (if new to Brooklyn Youth Soccer) Coaches Needed: Register to be a Coach by submitting the Coach-Volunteer Registration Form (Current form with “Revised 6-26-2014” at bottom). Forms: All forms can be downloaded from the League Documents Page @ brooklynyouthsoccer NOTE: Registration fees are NON-REFUNDABLE. A $15 late fee will apply to any request to register after July 26th or to any mail-in registration postmarked after July 28th. BYS reserves the right to reject late registrations based on space availability.
