Tallmadge Little League 2014 REGISTRATION When: Sunday, January 26, 2014 12 p.m. until 4 p.m. Where: Gioninos Tallmadge Little League Training Facility (directly across from the Hole) *ALL REGISTRATIONS MUST BE COMPLETED IN PERSON* INSTRUCTIONS FOR NEW TALLMADGE LITTLE LEAGUE PLAYERS: (ITEMS NEEDED AT REGISTRATION) 1. COPY OF EACH CHILD’S BIRTH CERTIFICATE 2. PROOF OF RESIDENCE: (One item from three or more of the following categories): Driver’s License, Voter’s Registration, School Records, Welfare/Child Care Records, Federal Records, State Records, Local (Municipal) Records, Support Payment Records, Homeowner or Tenant Records, Utility Bills, Financial Records, Insurance Records, Medical Records, Military Records, Vehicle Records, and Internet, Cable or Satellite Records.
