HOME HEALTH AIDE (HHA) Welcome House Home Health, LLC. - Westlake, OH Purpose: The home health aide enables people who are disabled, cognitively impaired, or chronically ill to stay in their homes by monitoring and recording the patient’s condition, providing support, personal services and teaching families. Description: The home health aide is a part-time entry level nursing position at Welcome House Home Health LLC. The starting pay is $12.50 per hour for time spent in training and starting at $25.00 for each home care visit. Essential Requirements: Current certification as a State Tested Nurse Aide in good standing from the Ohio Department of Health Performs job in a responsible and professional manner in accordance with established policies and training Responsible and reliable, arriving on time for assignments and maintains good attendance record Flexibility with scheduling English literacy and good documentation skills Good communication skills High school diploma or GED Age 21 years or older United States citizen or legal immigrant Valid Ohio Driver’s license with 4 points or less Personal motor vehicle and automobile insurance Able to work effectively as part of a team Satisfactory credentials, background check and drug test Able to lift up to 50 pounds, transfer patients safely and respond quickly to emergencies Desire to work with individuals with disabilities in the patient’s home Home Health Aide Job Duties: Provide healthcare in someones residence. Work under direct supervision of a nurse or physical therapist. Record and note clients condition and progress. Report changes to case manager. Check patients pulse rate, temperature, and respiration rate. Perform prescribed exercises and other physical therapy. Administer medications. Change dressings. Provide skin care. Massage limbs. Assist with braces and artificial limbs. Assist with medical equipment. Ensure equipment is functioning properly. Replace batteries on equipment.. Implement behavior plans. Teach self-care skills. Pick up medication. Transport patient to doctors office or hospital. Monitors patient condition by observing physical and mental condition, intake and output, and exercise. Assists patients by providing personal services, such as, bathing, dressing, and grooming. Helps patients care for themselves by teaching use of cane or walker, special utensils to eat, special techniques and equipment for personal hygiene. Helps family members care for the patient by teaching appropriate ways to lift, turn, and re-position the patient; advising on nutrition, cleanliness, and housekeeping. Records patient information by making entries in the patient journal; notifying nursing supervisor of changing or unusual conditions. Maintains a safe, secure, and healthy patient environment by following asepses standards and procedures; maintaining security precautions; following prescribed dietary requirements and nutrition standards; Updates job knowledge by participating in educational opportunities. Protects the home care agency by adhering to professional standards, home care policies and procedures, federal, state, and local requirements.
