Here we go again! The Social Services will hold a review. So will all the statutory agencies involved. Someone may get censored,someone may get retrained. No-one will be dismissed. The report from the review will most likely blame a "Breakdown in communication between the agencies involved." Recommendations will be made and new guidelines and "Robust protocols put in place to prevent a recurrence of these tragic awful events " Everyone involved will agree that this is a terrible tragedy and it must never, ever happen again! ......Until next time!
