I have a question regarding the new 2013 DBQ forms. My civilian Physical Therapist has a DPT which means a doctorate of physical therapy degree. Will the VA accept her performing the real testing and complete the VA form 21-0960M-9 to counter the lying C & P 3 to 4 minute exam??? The C & P man did not do the tests he checked that he did on the form... He LIED! But, the good news is my PT is willing to do the testing and complete the form for me. I am awaiting referral for NVCC fee based paperwork for the PT and for a total knee replacement on my S/C knee. The head ortho surgeon at Dorn sad he was putting in a request, but no indication appearing that it is even being considered my the powers that be, to approve out sourcing my surgery to a civilian entity that uses the 3 D printer technology available to make personalized replacement joints. I just dont want the lying C & P doctor to screw me out of this surgery paid for by the VA, but done by qualified specialists. Walking on crutches for 5 years has damaged my shoulder(s), especially my right shoulder, due to my S/c knee being the left knee. Rotor cuff tears (2) and Humeral stage three and 4 damage, now.
