I just came from the hospital, very tired after receiving 2 preterm babies twins 26 weeks. mother came fully dilated and delivered within 1 hour. she did not receive betametasone. 6 neonatologists were waiting for them in the labour room, 2 doctors and 4 murses. Each baby was managed by one doctor and 2 nurses. 1st one was a girl 620 gm and the 2nd one was a boy 700 gm. early nasal CPAP was tried for both of them in the labour room from 1st minute of life. Initialy they were ok but after 10 minutes they required 50 % oxygen through CPAP to maintain spo2. Both of them were intubated in labour room and transferred to NICU on portable ventilators and in portable incbators. In NICU, UVC were inserted immediately and started on 80 ml/kg/day Dextrose 10% with calcium gluconate 10 %, 2ml. CXR and AXR were done and UVC and ETT were in good position. the girl CXR was good, no HMD and oxygen was decreased gradualy to 25 %. the 2nd twin CXR showed sever HMD and his oxtgen requirement was 50 %, so Survanta 2.8 ml was given through ETT. whithin 10 minutes his saturation improved to 100 % and we were able to reduce his oxygen to 27 %. both blood gases and blood sugars were normal. I left both of them in good condition, vital signs stable and maintaining saturation. Planning to extubate them tomorrow. Any comments on the managment ???????
