I went through a similar struggle with my son. In 3rd grade his teacher sent home a form for me to fill out regarding my sons habits and behavioral patterns at home with a letter stating that the school suspects ADD based on his in school performance. I refused to fill out the form and brought it back to school. I requested that my sons teacher stop trying to analyze him and do what she is being paid to do by our tax dollars: Teach my son and if she does not want to teach my son or feels unqualified to do so, that the school move him to a different class and teacher. This stand I took for my son, lead to issues with the administration and I no longer felt my son had a chance at the school for an unbiased education. He had basically become a target. I withdrew my son from public school, also seeing that he was becoming extremely disinterested in learning and began His homeschool education. My son graduated from Public High School with Honors.
