IMMEDIATE OPENING Support Coordinator (Full Time 35 hours) Day Program Services Primary Role: Responsible for providing and coordinating social and rehab. services to developmentally disabled participants in any of the agencies related programs to include day program services, transportation, residential, respite, community based supports as well as assisting families with home health service purchases. ESSENTIAL FUNCTIONS INCLUDE: Providing individual informal counseling and crisis intervention as needed to each person assigned to his/her caseload. Coordination of ancillary services that an individual may require in order to achieve optimum independence. Provides orientation to new individuals to caseload. Reviews individual’s rights, responsibilities, safety and contents of the client handbook. Developing, in conjunction with other team members, the individual’s overall plan of care and finalizes the plan and purchase order for services in additional to any other required documents for processing. Interviews each individual prior to all individual service plan meetings and reviews the outcomes of the plan following its development. Monitors the progress of each individual’s overall service plan and recommendations. Facilitates and participates in meetings together with other team members, in order to establish or review an individual’s progress. May be required to assist participant at their place of employment or within the community this could include assisting a participant in completion of tasks/goals. Individual will participate internal incident review process and work with Human Rights Committee on any questions/concerns pertaining to those on the case load. Maintains on-going communication with parents/guardians/group homes. Maintain applicable case records in accordance with agency policies and procedures. Assists individuals’ with enrollment of necessary forms. May be required to provide assistance to individuals and their families concerning social security issues, or unemployment benefits. Participates in DDD licensing surveys. Must display empathy, compassion and treat all staff and individuals supported with respect and dignity. ADDITIONAL DUTIES INCLUDE: May be required to assist with transportation. Responsible for the safety and well-being of participants, may include transferring of individuals in and out of wheelchairs using a lift system and assistance in areas of personal care. Qualifications: Education: Degree in human service preferred or equivalent years of experience. Experience: experience working with DD population, experience working with social security, unemployment and or worker’s compensation programs, computer knowledge and ability to work collaboratively with multiple agencies to coordinate funding of tier package services, valid driver’s license, Must be insurable by agency insurance company.
