JOB VACANCY - MUST FILL IMMEDIATELY ER Nurse Case Manager – Full Time LPN Two New Position. Housed at the Archbold Memorial Hospital, Thomasville, Georgia. 36 hr work week – Monday through Thursday. Benefits include Paid Holidays, Accrued Personal and Sick Leave, and Marketplace Health Insurance. Two LPN Case Manager will coordinate follow-up services for patients of the Archbold Memorial Hospital. JOB QUALIFICATIONS: 1. LPN with active, non-restricted state license. 2. Two years of current clinical nursing experience required. 3. Minimum of 1 to 2 years’ experience with quality improvement, disease management, and/or primary care case management desired. 4. Experience with mental health / addictive disease services a plus. 5. Strong verbal and written communication skills. 6. Experience with data entry preferred. 7. Focused customer service skills and excellent problem-solving skills. 8. Ability to utilize critical thinking and problem-solve in a nonthreatening manner. RESPONSIBILITIES INCLUDE: 1. Identifies client resource needs, provides information on possible resources to clients, and makes referrals for clients, when applicable. 2. Identifies potentially unnecessary services and care-delivery settings and recommends alternatives, if appropriate, by analyzing clinical protocols. 3. Collaborates with healthcare givers in reviewing actual and proposed medical care and services against established coverage guidelines review criteria. 4. Manages network participation, care with specialty networks, care with medical providers, and transfers to alternative levels of care. 5. Recommends services for clients utilizing care alternatives available within the community and nationally.
