Job Title: Engagement Specialist Status: Full Time, Non-exempt Location: Outpatient Clinic Shift: Could include days, evenings, nights, weekends, and/or holidays as specified Report to: Director of Outpatient Services Description: The Engagement Specialist conducts telephonic, written and face to face outreach and retention services to boost program participation (no-show rate reduction) for Mental Health and Substance Abuse patients admitted to the Credo Outpatient Services. The Engagement Specialist works closely with the Care Coordinator, clinical directors, and primary counselors to identify, locate and engage patients who are not attending their scheduled sessions. The Engagement Specialist assists patients with removing barriers to treatment, motivates patients to engage in their health and supports them throughout the process. Essential Job Functions: • Establish telephonic, written and face to face contact with admitted patients in order to facilitate engagement in treatment and improve retention. • Utilize internet databases, health record systems, and other resources for administrative record keeping. • Utilize Electronic Health/Medical Record system(s) of assigned Health Home(s) and maintain ongoing clinical documentation and records upholding all 42 CFR/HIPAA regulations and confidentiality protocols to record each interaction with every patient served. • Coach, motivate and support patients through treatment process to improve retention and increase potential to be successful. • Provide direct care services and facilitate group sessions. At least 50% of the time will be dedicated to direct service delivery. • Assist Care Coordinators in securing and identifying needed referrals to community and network with medical, behavioral health and social assistance providers through telephonic, written, and/or face to face outreach, research, and maintenance of the program resource directory. • Provide follow up services via telephonic or face to face engagement with clients and service planning partners as needed. • Provide oral and/or written status updates regarding clients’ progress, regress, and needs to responsible counselors, care team, and/or supervisor, and any service providers, legal mandate, or other care plan affiliates as needed to assist the program and clients. • Attend meetings and case conferences in the agency and community • Attend staff meetings, in-service and conference/trainings as assigned • Convey information inter/intra agency • Be sensitive and responsible to public relations within the community • Perform other duties as assigned Note: The above statements are intended to describe the general nature and level of work being performed by individuals assigned to this position. Minimum Job Qualifications: College Degree and experience in Substance Abuse and Mental Health, LMSW preferred, licensed mental health counselor in the state of NY accepted And Support the goal of abstinence from mood altering substances unless prescribed by a physician knowledgeable of addiction And Must subscribe to a Professional Canon of Ethics. Preferred: • Three (3) years of job-related experience providing medical, mental health or substance abuse-focused services to individuals with chronic medical conditions and/or severe and persistent mental illness • Excellent interpersonal, organizational, writing and computer skills • Ability to engage clients and diffuse difficult situations • Experience working in the medical social work or community mental health field • Knowledge of Integrated Behavioral Health in the community healthcare system • Knowledge of applicable federal, state, and county regulations Submit letter of interest and resume by October 31, 2014 to: Human Resource Director Credo Community Center 595 W Main St Watertown NY 13601 Fax: 315-755-2538
