LOW INTENSITY SUPPORT SERVICES The Low Intensity Support Services (LISS) program grants eligible applicants up to $2000 for identified services and items to support identified needs. The DDA distributes LISS funding twice during the fiscal year (July 1 – June 30), in two rounds. Round 2 begins January 1 – 31, 2015 Please do not send requests before January 1, 2015, they will be returned. NEW LISS REQUEST FORM Beginning with Round 2, the new request form is preferred; please click here to download the new form. -Please note: incomplete requests will be returned. Please be sure to include the required information listed in Sections 3 & 4 with your request. -To view a sample of a completed request form please visit the DDA website: dda.dhmh.maryland.gov/SitePages/liss.aspx. WHERE TO SEND REQUESTS The LISS agencies accept requests by mail, and in-person delivery, and email January 1 –January 31. Please do not fax requests.
