MBBS/BDS ADMISSIONS in Public Sector Medical & Dental Institutions of the Punjab Session 2013-14 Procedure for Submission of Admission Forms 1. Prescribed Admission Forms and Prospectus shall be available Free of Cost at the University of Health Sciences Lahore, Nishtar Medical College, Multan, and Rawalpindi Medical College, Rawalpindi, from 21st October to 26th October, from 09:00 am to 02:00 pm. The candidates will have to provide the attested copies of their SSC, HSSC and Entrance Test result cards and domicile certificate to get the Admission Form. 2. The candidates shall submit the duly filled Admission Form along with following documents, at the same centre from where they get the Admission Form, from 21st October to 26th October, from 09:00 am to 02:00 pm: (a) Three (03) attested copies of SSC or equivalent certificate / result card. (b) Three (03) attested copies of HSSC or equivalent certificate / result card. (c) Three (03) attested copies of Domicile Certificate of the candidate. (d) Three (03) attested copies of character certificate from the institution last attended. (e) Three (03) attested copies of CNIC of Father/Guardian. (f) Four (04) recent passport size colour photographs; one attested on the front and three attested on the back.Admission Schedule i. Availability of Admission Forms and Prospectus 21st October to 26th October, 2013, 09:00 A.M. to 02:00 P.M. ii. Submission of Admission Forms (Open Merit, underdeveloped, Disabled, Cholistan) 21st October to 26th October, 2013 09:00 A.M. to 02:00 P.M. iii. Medical Board of Disabled Candidate 4th November to 7th November, 2013 iv. First Selection List (MBBS) 16th November, 2013 v. Commencement of classes in medical colleges 2nd December, 2013 vi. First Selection List (BDS) 24th December, 2013 vii. Commencement of BDS classes 15th January, 2014 (g) Original Fitness Certificate issued by a registered medical practitioner / Government Medical Officer. (h) Original Affidavit (Annexure I) on Rs.20/- stamp paper as per specimen given on UHS website. (i) Disabled candidates shall submit a certificate issued by a specialist working in Government Hospital describing the nature of disability. The disability, however, shall be verified by a Medical Board. (j) For Under-Developed District seats, candidate will have to submit original certificates of the schools and colleges of the respective district from where he/she has studied from class one to twelve. These certificates must be verified by respective District Education Authorities and countersigned by the District Coordination Officer of the district. These candidates shall also have to submit a Surety Bond (Annexure II) the specimen of which is available on UHS website. (k) For Cholistan seat, the candidates will have to submit a certificate of Cholistan Development Authority clearly stating that the candidate is Cholistani by birth and has actual residence in Cholistan. The candidate must have passed HSSC exam from Bahawalnagar, Bahawalpur or Rahim Yar Khan. IMPORTANT CONDITIONS: 1. The candidate shall have to rank the colleges in the Admission Form as per his/her own choice. The order of preference once given shall be final and cannot be changed subsequently. This condition is mandatory and neither any subsequent change is entertainable nor any exceptions will be made. A candidate will never be considered for a college, which he/she has not named in his/her list of choices. 2. There shall be a single Admission Form for MBBS/BDS, Open Merit, Reciprocal Seats, Under-Developed District seats and Disabled Candidates’ seats. 3. The candidates shall submit Admission Form by hand. Forms received by Post shall not be entertained. No form will be accepted after 02:00 pm on 26th October, 2013. 4. Documents submitted with the Admission Form will be considered final. Revised result cards shall not be entertained once the form has been submitted. The documents must be complete in all respect. Receipts of domicile etc, shall not be accepted at this stage. 5. Any candidate found to have submitted forged certificates or fake documents at any stage of admission, shall be permanently debarred from admission to any medical / dental college. 6. Candidates of FATA, Azad Jammu & Kashmir, Gilgit-Baltistan, Baluchistan and those with Foreign nationality should contact their respective governments/ nomination boards/ organizations for admission against seats reserved for them in medical / dental colleges of the Punjab. UHS shall not entertain any direct application for admission against these seats. 7. The result of Entrance Test 2013 can be downloaded from UHS website.
