MRCP(UK)-1 PREPARATION By Sumanta Sirs Academy Respiratory Medicine Question-01 A 72-year-old man comes to the Respiratory Clinic with increased shortness of breath and wheeze. His GP has tried him on a regular salbutamol inhaler, but this has had little effect on his symptoms. Unfortunately he continues to smoke 30 cigarettes per day and has done for the past 50 years. On examination, his blood pressure is 155/85 mmHg and his body mass index (BMI) is 21 kg/m2. He has respiratory changes consistent with chronic obstructive pulmonary disease (COPD), including coarse wheeze throughout both lung fields and crackles. Investigations show: haemoglobin 12.3 g/dl, white cell count 6.7 × 109/l, platelets 201 × 109/l, sodium 139 mmol/l, potassium 4.9 mmol/l, creatinine 130 µmol/l, forced expiratory volume in 1 second (FEV1) 50% of predicted, transfer factor reduced. Which of the following most closely correlates with mortality in COPD? A- FEV1 B- Peak expiratory flow rate (PEFR) C- Residual volume D- Total lung capacity
