Our official charity partners The Kaleidoscope Plus Group are playing an active part in the new The Community Offer project, which targets elderly and vulnerable people across Sandwell to prevent unnecessary admissions to acute health and social care services. The Community Offer utilises and develops the excellent preventative services offered by voluntary and community organisations. The Kaleidoscope Plus Group will be working with residents specifically living or registered with GP’s in West Bromwich Central, Lyng and Greets Green and are keen to hear from people who believe they could benefit from this exciting new service.
