PRC INITIAL REGISTRATION FLOW :) 1. Buy documentary stamps at documentary stamp window. Then proceed to prc registration division. 2. Present your NOA to the officer who are taking care of printing of OATH FORM. 3. Then after you took your printed oath form, proceed to the VERIFICATION WINDOW and present your OATH FORM. You will be given the PRC EXAMINATION and REGISTRATION CARD. 4.Proceed to CASHIER to pay the initial registration. Fill up the necessary details to be intstructed by the officer. For us in PRC MAIN, we fill up the back the portion with the DATE, OR NO and the AMOUNT PAID. 5. Then after you fill up the PRC EXAMINATION and REGISTRATION CARD the officer will collect it. After collecting, the officer will give you the PRC REGISTRY SHEET. At that sheet, your signature and thumbmark is needed. 6. After you fill it out the Registry Sheet, proceed to REGISTRATION WINDOWS. There they will need your PRC EXAMINATION and REGISTRATION CARD, 1 DOCU STAMP, REGISTRY SHEET. After you present the present the requirements the claim stub will be given to you. Notes: * Please write your name and profession, LICENSE NO. at the LEFT UPPER PORTION OF SHORT BROWN ENVELOPE. Whereas, the SEQUENCE NO. is on the RIGHT UPPER PORTION. * The other documentary stamp and the PASSPORT SIZE ID Picture are the things that will be put on the SHORT BROWN ENVELOPE. * PASSPORT SIZE ID PICTURE is ONLY NEED *PRC ID AND COR WILL BE CLAIM at OATH TAKING as stated in the PRC CLAIM STUB. *1050 - INITIAL REGISTRATION FEE *Please follow your REGISTRATION SCHEDULE and to where you apply for BOARD EXAMINATION to avoid problems to occur.
