Parent Educator Reports to the Lead Parent Educator MISSION: Gads Hill Center, a social service organization established in 1898, creates opportunities for children and their families to build a better life through education, access to resources and community engagement. JOB SUMMARY: The Parent Educator provides education and family support to pregnant mothers and families with children ages birth to three enrolled in the Prevention Initiative Home Visiting program. The Case Manager / Home Visitor is responsible for planning and implementing education activities for assigned families during individual home visits, planning and engaging families in regular socializations and conducting outreach and enrollment activities to maintain full program enrollment. In addition, this individual will complete a family assessment and develop an Individual Family Service Plan in collaboration with each family to develop a plan for helping families meet their needs and goals. DUTIES AND RESPONSIBILITIES: Education 1. Develops and implements educational plans and activities to meet each individual familys needs according to the Parents as Teachers curriculum. 2. Work with assigned families on a minimum of two monthly one-on-one 90 minute home visits, and during monthly parent group meetings. 3. Assists parents in developing parenting skills, helping them to become the primary family educators and their children’s first teachers. Reinforces this concept with practical suggestions for its development. 4. Conducts health checks during each visit, documenting findings and following-up as needed. 5. Encourages parents involvement in and supports planning of monthly Parent Committee meetings 6. Administers and interprets development assessment tools to assigned children within the required time frame, maintaining detailed, written documentation. 7. Serve as the liaison between parent groups and Support Services Coordinator and/or other designated staff. 8. Participate and assist with program self-assessment, site monitor visits, developmental screenings and Chicago Public Schools or Early Intervention evaluations. 9. Inputs required assessment and any other required data into GOLD and ETO database. Case Management and Family Support 1. Oversee and maintain all social service, health, nutrition, mental health, etc. files and data updated in ETO and any other database in order to ensure timely referrals and follow through. 2. Creates an annual Individual Family Service Plan within 30 days of enrollment for each assigned family and updates it at least once every six months, maintaining confidentiality of family records and information. 3. Provide direct and indirect services for referrals aligned to family goals. Document all referrals and family contact. 4. Provide support and education to pregnant mothers about child development and ensure that pregnant mothers follow through with prenatal medical and dental visits and other resource needs. 5. Assists families in identifying and accessing resources, fully utilizing available community resources, and making referrals, documenting service and following up as needed. 6. Work with supervisor, Support Services and Disabilities Coordinator, other Gads Hill staff, mental health consultants, CPS staff and other pertinent individuals in meeting the needs of special needs children as it relates to SRT meetings, referrals and follow up to ensure children and families are being provided with timely and thorough services. 7. Engage families in parent groups and other parent involvement opportunities. 8. Assist family support team to partner with community agencies and form linkage agreements. 9. Maintain community resource guide to assist parents in identifying and developing local community resources. 10. Work with Support Services Coordinator to identify and meet training needs of parents. 11. Collaborate with other staff, volunteers, interns, and/or neighborhood organizations with on-going program implementation and in developing special events. 12. Attend regular family support meeting, parent meetings, and other meetings as necessary. 13. Perform record-keeping and other reporting functions as required by funder and submit all agency reports in a timely manner. Maintain accurate records and reports as required by agency policies and program standards. 14. Assists with the recruitment of children for the program. 15. Ability to work flexible hours including occasional evenings and weekends as required. 16. Performs other duties as assigned. In addition to the responsibilities listed above, all Gads Hill Center employees are responsible for the following: 1. Participate in agency outreach and recruitment events, including evenings and weekends. 2. Participates in supervision meetings and workshops within the agency and outside (if applicable). 3. Participates in civic engagement activities that advance the mission of Gads Hill Center. 4. Advance parent leadership development. 5. Performs other duties as assigned. QUALIFICATIONS: 1. Bachelor’s Degree in Early Childhood Education, Social Work or related field and one year previous direct service experience with low-income families required. 2. Previous early education and family support program experience preferred. 3. Bi-lingual Spanish/English required 4. Excellent interpersonal communication and organization skills. 5. Ability to pass a criminal record and child abuse/neglect registry check. 6. Valid Driver’s License and use of a dependable insured car required. 7. A passion for the mission of Gads Hill Center.
