Patient with insurance of their choice: I burned myself and just want you to check it out Clinic: When was your last tetanus shot Patient: I think it was almost ten years ago but not quite maybe 7 Clinic: We dont have a record here Patient: Wouldnt you be able to get it from my primary care physician? Clinic: Not unless you sign a release form Patient: Oh, okay then I will just have my doctor do it. What a pain Clinic: We can still do a TB screen for you if you want Patient: okay might as well while I am here we always have to stay updated with that Calls Doctor...Patient with insurance of their choice: I would like an appointment to get a tetanus shot I think its time nd is required by my employer. My employer is required by law that they have these records for ALL employees. Clinic: We can do the Tetanus shot for you do you need the TB screen also? Patient: No, I had that done. Just the tetanus At the appointment: Doctor: You havent been here in a while I am required to have current records on all patients that I treat Patient: okay what do I need? Doctor: Well we do a physical Patient: okay Doctor: Well while you are here when was your last blood test Patient: For what? You mean blood tests are a requirement? What are you testing for? Doctor: Well I like to report that all of my patients have had certain screenings Patient: No thank you. I do not wish to participate in the study Doctor: It wont hurt to know and I cant treat you without complete information Frustrated....Patient: Like what? Doctor: We check for levels in your blood Patient: Levels of what? Doctor: Your blood count and then I like to order screens Patient: For what like HIV? Doctor: We can certainly do that for you Patient: I already had that why dont you have those records? I put your name on every form for appointments and you dont even have a record of my last tetanus shot. Doctor: Well we dont automatically get your records Patient: Well that is a problem because I am not going to have the same examinations repeatedly. They already drew blood for my employment screening where are those results Doctor: How current is that Patient: Nothing has changed Doctor: Have you had a mammogram Patient: I came in here for a tetanus shot and thats all. Doctor: Okay part of the physical is a breast exam Patient: Why do I need a breast exam for a tetanus shot? Exams breast... Doctor: Okay I am going to send you over to X breast center Patient: WHAT?!! I just need a tetanus shot Doctor: You are very healthy and would be a good candidate Patient: If I am very healthy then I dont need to go over there Doctor: Well I felt a lump..its inconspicuous but they can confirm that Patient: Without a mammogram first? and mammograms studies show a high rate of false positives so patients then undergo other procedures for no reason just to find out they dont have anything. I have no risk factors or symptoms no thank you. However I would like to know if I might be diabetic. I was gestational diabetic Doctor: I need to make sure you dont have breast cancer Patient: I dont have it no thank you Doctor: How do you know Patient: No risk factors, no history and no symptoms Doctor: I felt a lump Patient: lumps are normal especially at my age when the breast tissue changes and I was a nursing mom Doctor: How do you know so much Patient: Oh I am a radiographer too... Doctor: Why didnt you tell me Patient: I forget but I know alot more than you think Doctor: okay I just encourage you Patient: I know you have a checklist for your records... just do what you have to Doctor: I will prescribe you a high dose of vitamin D which is known to be effective in treating breast cancer Patient: mmhm Doctor Follows up with blood test results: Everything was excellent but your vitamin D levels were very low Patient:mmmhm... *** true story
