Please say a New Years prayer for my mom. I just learned she has been given too much Coumadin AGAIN. When at KingsBrooke she was given A double dose and her INR (thinness of blood) went to 3.9. They withheld the Coumadin until the number fell in the safe goal range of 2.4. Last week I learned nurses at Woodland Oaks misunderstood the dr orders and gave mom 9 mg (she was supposed to have 4 mg one day and 5 mg the next thru the week and they thought the dr mean 9 mg at one time each day). Her INR rose to 3.3 (danger zone is over 3). Tonight moms INR was 4.5---extremely dangerous (no wonder she is so pale and cant keep her eyes opened). Apparently the Coumadin was restarted when moms INR numbers were still too high. I am wondering if I should bring her home and do the Coumadin therapy myself??? She is there for rehab. I can give her meds myself. The PT was what we needed for rehab.
