Posting....MDCH Job Posting: Public Health Consultant 2A-Chronic Disease Network State Coordinator. This advanced state level position will serve as the statewide consultant and coordinator for up to six community-based Chronic Disease Coordinating Networks (CDCNs) that are funded by a CDC 1422 grant focused on state and local partnerships to address chronic disease in high burden areas. This position will be responsible for providing expert advice, consultation and technical assistance and oversight to funded community-based agencies to ensure they have the capacity to engage key partners in planning and implementing 1422 required evidence-based programs and strategies to prevent and reduce obesity, as well as prevent and increase control and self-management of diabetes and hypertension among high-risk populations. Critical project management responsibilities of this position include contractual oversight and provision of technical assistance to the CDCNs on program development, implementation, evaluation and fiscal management as needed. In addition, this positon has lead responsibility for the coordination of program specific technical assistance needed from the Cardiovascular Health, Nutrition, and Physical Activity and Diabetes and Chronic Diseases Section and the 1422 Evaluation Team.
