Preliminary Report of Overland Ilorin Incident. As part of the commitment to ensure time bound performance of our key functions and transparency, find below part of the Preliminary report of the Overland incident of 29th November issued by the Accident Investigation Bureau PRELIMINARY REPORT OF SERIOUS INCIDENT INVOLVING ATR 72 BELONGING TO OVERLAND AIRWAYS LIMITED WITH REGISTRATION 5N-BPG WHICH OCCURED AT ILORIN AIRPORT KWARA STATE ON THE 29TH OF NOVEMBER, 2014. REGISTERED OWNER AND OPERATOR: OVERLAND AIRWAYS LTD. PLACE OF INCIDENT: ILORIN AIRPORT, KWARA STATE DATE AND TIME: 29TH OF NOVEMBER 2014 AT ABOUT 16:44 hrs. LOCATION: ILORIN AIRPORT 1.0 FACTUAL INFORMATION 1.2 HISTORY OF FLIGHT: The Overland Airways flight (OLA 1186) departed Abuja for Ilorin at about 15:45hrs had the first VHF contact with Ilorin (DNIL) at 16:01hrs with 59 Passengers and 4 crewmembers, three hours and forty minutes endurance, estimating Ilorin at 16:30hrs. At 16:14hrs OLA 1186 requested for descent and was granted descent to 6,000ft and was asked to report 30 Nm, at 16:21hrs OLA1186 requested for further descent and a straight-in VOR approach runway (RWY) 23. OLA 1186 was asked to stand-by due to traffic, HS 125 with registration N827SA from Lagos to Ilorin on finals RWY 05 and 16 NM. At 16:27hrs the traffic was sighted at the threshold of RWY 05 and OLA 1186 was instructed to continue flight and intercept centerline RWY 23 and descend to 3,500ft and to report RWY 23 in-sight At 16:32hrs OLA 1186 reported maintaining 3,200ft requesting to join the hold overhead ILR VOR, the Tower corrected OLA 1186 that the initial altitude is 3,500ft and not 3,200ft as reported. OLA1186 reported maintaining 3,500ft as cleared and asked tower to stand-by, that they are having problems and will get back to tower. The Tower promptly alerted the Fire Service to be on stand-by. At 16:36hrs OLA 1186 requested for the position of his traffic; the only traffic then was the DA 40, training aircraft operating at area 4C - south-east of the field at 4,000ft and 15NM. OLA 1186 then requested to come in on RWY 05 on priority landing because of their problem ‘Hydraulics’. The aircraft was cleared to land without any traffic in sight. The Fire Service was informed of OLA 1186 hydraulic problem, was advised of aircraft position and RWY in-use. At 16:41hrs OLA 1186 reported 6NM final RWY 05 in-sight and was cleared to land RWY 05, wind 120/05 and to check three greens, the crew responded and acknowledged the clearance. The Tower confirmed the gears down. At 16:44hrs, OLA 1186 on landing roll after passing link 2, veered-off RWY 05 to the right, coming to a stop at a distance of about 171ft into the grass area with the nose wheel collapsed under the aircraft and about 40ft from the edge of RWY 05. However the tail of the aircraft was only about 12ft from the edge of the runway. The aircraft engines were shutdown with the fire switches, as the engine control levers were jammed. Evacuation was initiated; crew and passengers were evacuated without injuries or fatalities. The controller triggered the crash alarm, Fire Service arrival was prompt and Foam was applied to the port side due to smoke around the port engine while another fire truck embarked on cooling the fuselage and wings of the plane with water. 1.2INJURIES TO PERSONS INJURIES CREW PASSENGERS OTHERS FATAL NIL NIL NIL SERIOUS NIL NIL NIL MINOR/NONE 4 59 NIL 1.3DAMAGE TO AIRCRAFT: The aircraft was slightly damaged, with the nose wheel collapse under the aircraft and affecting engine control. 1.4OTHER DAMAGE: There was no other damage. 1.5PERSONNEL INFORMATION 1.5.1PILOT FLYING Date of Birth - 30TH September, 1959 Nationality - Zambian Licence No. - ATPL 4830 Medical Expiry - 13TH December, 2014 1.5.2PILOT MONITORING Date of Birth - 04TH January, 1981 Nationality - Nigerian Licence No. - CPL 6011 Medical Expiry - 14TH April, 2015
