TALKS INCLUDE: RIGHT COLECTOMY FOR CANCER: THE NEXT FRONTIER FOR COLORECTAL SURGEONS? Pascal Gervaz (Switzerland) Since the 1990s, wide adoption of TME has resulted in dramatic improvement of outcome for rectal cancer patients. By contrast, the technique of surgery for colon cancer has remained virtually unchanged in Europe. We searched the Geneva Tumor Registry Database, and observed a steady increase in survival for patients with left-sided, but not for right-sided tumours. While, in 1980, 5-year specific survival was identical for right and left colon cancers (49% vs. 47%), outcome became markedly superior for left than right colon cancers in 2006 (75% vs.60%); thus, right colon cancer has been left behind, and patients with proximal tumours should be considered the next target for implementing better surgical techniques. In this talk, Dr Gervaz will review the modalities and potential benefits of right complete mesocolic excision (CME) with extensive (D3) lymphadenectomy for right colon cancers. TRANS ANAL PROCTECTOMY: AN ALTERNATIVE APPROACH? Prof Philippe Rouanet (France) Trans-anal proctectomy consists in performing a complete anterior resection of the rectum from the anus to the pouch of Douglas. Posteriorly the surgeon can reach S2 S3 but the anterior dissection is really the cornerstone of this procedure. It is different from the perineal first approach which only dissects the endo anal part of the lower rectum and it is not always synonymous with a NOTES procedure which can be performed with a classical abdominal approach. What are the main benefits of this technique? Come and listen to Professor Rouanet give a detailed description and indications of this Bottom Up technique. HOME, SWEET HOME. MAXIMIZING SURVIVAL IN COLORECTAL CANCER SURGERY. Miss Katie Adams (UK) Should we see and scan our patients after they go home following cancer surgery? Can routine surveillance after colorectal cancer resection improve outcomes? How often should we scan or scope our patients and for how long? Is there enough evidence to justify the time and expense? Could we possibly be harming our patients psychologically by subjecting them to needless hospital visits? Ms Adams will discuss the risks and benefits of post-treatment surveillance, the risks and pitfalls of hospital based follow up, as well as comparing survival rates amongst different international follow up regimes.
