SUPRACLAVICULAR BRACHIAL PLEXUS BLOCK this new block of today list done for plating radius bone with tourniquet ,it done by mixture of 10 ml 2% lidocaine with 10 ml 0.375% bupivacaine,the duration of surgery was 110 minutes. see here my colleagues how the brachial sheath is hard and see the upper injection when the spread firstly was above the sheath then i pierced the sheath and see how the LA spread inside with nice appearance of divisions and suprascapular nerve, and see the pocket injection just rise up the plexus but not complete spread to entire plexus and that might lead to much sparing that depend to volume and rate of injection and how the sheath is hard.....some patient have thin sheath and the hard one is the outer layers surround it which represent the anterior facial sheath of middle scalene and posterior facial sheath of anterior scalene, but some patient have hard cervicoaxillary sheath as in this patient. this block is the most popular one in my hospital around 100 to 150 SCB per month to 1500 to 2000 per year.
