Senior Case Manager, Supportive Housing Coordinator Boston Senior Home Care - Boston, MA 02111 Supportive Housing is a program of BSHC that assists low income residents who reside in elderly/disabled housing complexes to age in place and make lifestyle changes that improve the quality of their life. The Supportive Housing Coordinator assists residents with appointments, community referrals, and arranging transportation and translation services, and is responsible for arranging and coordinating all supportive housing program activities. The SHC works closely with the buildings Resident Services Coordinator to identify residents who might be in need of Home Care or other services, and provides assistance with tenancy issues, as appropriate, to help avoid eviction. The SHC may also be required to provide ongoing case management to residents who receive services from Boston Senior Home Care. & The SHC must possess advanced skills in consumer assessment and be able to assess the physical conditions of the consumers home as well as the consumer. ( Interpersonal flexibility, initiative, teamwork, a customer service attitude and excellent oral and written communication skills are key qualifications for this position as are solid organizational skills) & The incumbent is responsible for both identifying issues that arise in a building and effectively working with all partners. The SHC will work with ASAP and non-ASAP consumers and any resident regardless of age. The Supportive Housing coordinator will be solely based in select buildings in Boston Senior Home Cares catchment area. Salary Range: Starting at $39,278 Requirements 1. May carry a caseload that includes, but is not limited to, EOEA based programs. In determining whether SHC will carry a caseload, consideration will be given to the number of housing units in the building, and the percentage of younger disabled residents. 2. See 11 3. Serve as a resource person for the assigned Supportive Housing building by providing knowledge on consumer and government resources etc. to both ASAP and non-ASAP residents. 4. Develop and maintain positive relationships with Supportive Housing partners including but not limited to the Housing Authority. 5. T o be trained on Evidence Based classes and conduct trainings to the residents. 6. Disseminate information to residents by creating and maintaining monthly calendar of events and newsletter. 7. Conduct assessments and Intakes as assigned. 8. Oversee block authorization process; assist in determining cap level, analyzing monthly cluster reports and communication with vendor as necessary. 9. Assist Program Manager in developing and implementing outreach activities to increase enrollment for all BSHC care management programs. . 10. Plan and direct on and off-site social activities, workshops and educational programs. Examples include films, arts and crafts, games, outings and wellness activities. Develop and implement procedures for engaging consumers in planning, implementing, leading and participating in activities. 11. Perform other duties as assigned by Team Manager. QUALIFICATIONS: & Six months or more years experience in elder care case management required. BS or BA degree required, and licensure preferred. Must have computer experience, and be able to perform basic data entry and effectively utilize MS Office Suite. Incumbent must be able to carry a 15 to 20lb load to consumers homes, must be able to travel to the community to perform visits at consumers home (may have limited accessibility), and travel under all weather conditions.
