^^^***Shared from a friend! Please read and share!***^^^ Please Share! I have a HUGE story. I went to the health department today. Was called in to do my paperwork. Was told they dont take my private insurance. Then the lady told me I had to fill this form out. As I started filling it out I just read the first line... 1.) Directions to Home (Leaving Health Dept. To Home)... I said, Thats strange, why would the health department need directions to my home? She responded, In case of an emergency, we can know how to get to you I responded by saying, Wouldnt it be kind of hard to go to my file and pull a sheet out in case of an emergency? Have yall actually used this? She responded, Not really. We havent used them. We keep them for events such as chemical warfare and other similar events. .... SCREEECH.... WHAT?! Now this lady was talking calm like this is just normal. I continued down the sheet... 2) The number of miles from the health department to my home?! 3) The Description of my Home including the color, shutters, house number, mailbox color?! 4) Distinguishing features of my home such as lawn ornaments?! 5) Family vehicles? Along with their description and color? 6) Houses or businesses nearby?! 7) Time of day most likely to be home?! Now... As I went down this list all sorts of alarms are going off... I asked the lady, I really dont feel comfortable filling this out. Is it required If I want to receive services? Can I refuse to fill it out? She responded, It is required. You have to fill this out. Every person who comes here does. This is a HUGE story and we need to push it into the media. I have gone to the public health departments for years... Never had such a form to fill out. This is insanity.
