Some of the highlights include: Bridging the gap between hospital and home Setting up follow-up appointments prior to discharge Following the patient through the post-discharge process Creating an interdisciplinary team Providing information discharge Enhancing patient education Recording discharge instructions so the patient has a copy when they return home Providing links to helpful videos on the hospital site Providing home visits to check on medication adherence, making sure the patient has a place to live and a support system, and making sure the patient is stable What are you doing to reduce avoidable readmissions?
